Equipment Loan Library
The Equipment Loan Library service is available to all North Dakota residents
with disabilities, their family members, and the professionals that support them.
Use the Equipment Loan Library to:
• Explore a variety of assistive technology (AT) devices
• Try AT devices before you buy
• Replace AT devices in for repair

Policies
IPAT operates a statewide short-term assistive technology equipment loan program known as the IPAT Equipment Loan Library. The loan library is available to North Dakota residents of all ages with all types of disabilities and/or those family members and professionals who work with them. No one is denied equipment based on their age, race, type of disability, income, or geographic region of the state.
The loan library offers a wide range of devices for short term loan, including: communication; telecommunications; computer access; learning/developmental equipment; vision; health, safety and daily living; hearing; switches; seating, positioning, and personal mobility; and recreation. The purpose of the equipment loan is to meet the
on-going need for exploration, trial use, AT funding justification, and for individuals whose own devices are in for repair or not working. All loan requests are accepted on a first-come, first-served basis.

Procedures
1. If the requested device is currently available, the IPAT loan librarian will complete an Equipment Request Form and send it to the borrower, via FAX or mail. This request form identifies who is requesting the equipment, the person it is being requested for (if different from the requestor), mailing address, telephone number, the
equipment inventory number and device name, rental fee, shipping charges, delivery option (pick-up or send out), and total cost.
2. After receiving the request form, the requestor is to review it, sign it, and return it to the loan library, along with appropriate payment. If requestor has a purchase order or claim number, this is to be noted on the equipment request form for invoicing purposes.
3. Once the signed form and payment (if applicable) are received, the device will be shipped or prepared to be picked up.
4. If the device is not currently available, the requestor's name will be placed on a waiting list if they so desire. If the loan library does not have the device in its inventory, the requestor will be referred to the appropriate vendor or other resource.
5. The loan library device loan period will be six weeks from the shipping date, with extensions granted on a
case-by-case basis, using the loan library extension policy, with appropriate fees to be charged. This extension policy requires a request for an extension from the requestor prior to the expiration of the original
loan period.
6. Failure to return the equipment to the Loan Library will subject you to legal action. Charges may be brought against you according to North Dakota Century Code 27-07,1-17.

Rental Fees
Fees range from $15.00 - $45.00. Devices will be shipped UPS with a pre-paid return label. Borrower is
responsible for shipping charges. A loan period is up to 6 weeks. Late fees will be assessed weekly at the rate
of the loan for each week the item is late.
